Physical Exams
It is preferred that your child's doctor perform their physical exam. However, if you want them to have their physical exam done in school, please fill out the form below.
- Physical exams are required for grades K, 1, 3 as well as for new entrants.
- Physical exams are required yearly for students with an IEP or 504, regardless of grade.
- Physical exams should include allergies, medical conditions, hearing, and vision screenings.

Medications
If your child requires medication while at school, please follow the guidelines listed below. Your doctor may use their own letterhead, or you may print out the provided medication form.
- All medications must be brought to the school nurse by a parent/guardian.
- All medications must have a doctor's order and signed parental consent (this includes all over-the-counter medication).
- It is the parent/guardian's responsibility to refill/provide the nurse with the medication as needed throughout the year.

Immunizations
All students must follow NYS immunization guidelines in order to remain in school.
- Students must meet all immunization requirements within the first 14 days of the school year (30 for out-of-state) in order to remain in school.
- Students who do not meet the NYS immunization requirements will be excluded from school and may not return until guidelines are met.

Illness/staying home from school
The following guidelines should be used when deciding to keep your child home from school. However, this is not a complete list. Please call the Health Office with any questions at 845-534-8009 ext. 2010.
- Students with a temperature of 100.0 or greater must remain home from school until their temperature is below 100.0 for a minimum of 24 hours without the use of medication.
- Students with vomiting that is unrelated to food or motion sickness should be kept home from school.
- Students with eye discharge and redness should be evaluated by a doctor before coming to school.
- Students who are on any kind of antibiotic (oral, cream, drops) for a contagious illness/rash/infection (ex. strep throat, pink eye) should stay home from school for 24 hours following the start of the antibiotic.
- Parents must have a plan in place to pick your child up from school within 30 minutes should they become ill at school and need to go home.

Injuries/Return to Play
Please refer to the following guidelines for students with injuries.
- All students with physical injuries that require any kind of wrap, brace, cast, splint, etc. as well as crutches will not be allowed to participate in PE and recess.
- Students who have a doctor's note stating that they may not participate in physical activity will not be allowed to participate in PE and recess until a clearance note is provided from the doctor. The clearance note must state 'may return to activity with no restrictions' and must have the date that they may return.
- Parents who feel their child cannot participate due to minor injuries may request this for up to two consecutive days. After two consecutive days, if the parent still feels that their child cannot participate, a doctor's note must be provided. This includes both PE and recess.
- If the school nurse sends your child home with an injury, it is recommended that you follow up with your child's doctor.
- Parents must have a plan in place to pick up their child within 30 minutes should they get injured at school and need to come home.

Allergies
Please refer to the guidelines below for food allergies. For all other allergies that require emergency medication, please see the medication section.
- Cornwall Central School District is 'Allergy Aware'.
- If your child has a food allergy that needs special attention, please print and fill out the 'Allergy Aware' packet and return it to the school nurse.
