Important Notice Regarding Student Injuries
Please be advised that all student injuries must be reported to the Health Office.
Students are not permitted to attend school using crutches, wheelchairs, braces, or other medical equipment unless a doctor’s note has been submitted to the school nurse in advance.

Student Health & Emergency Contact Information
To help us best support your child’s health and learning needs, please notify the school nurse of any medical conditions or current medications your child is taking.
You can contact the Cornwall Central High School Nurse by phone at 845-534-8009, ext. 5010.
Additionally, please ensure the school has your current phone numbers and emergency contact information on file. It is essential that you immediately inform the school of any changes to your emergency contacts so we can reach you quickly if needed.
Your cooperation is greatly appreciated in helping us keep your child safe and healthy at school.
Absence Due to Illness

Attendance and Health Notifications
If your child will be absent from school, please call the attendance line at extension 5009 to report the absence.
Additionally, if your child experiences a serious medical condition that requires restrictions or special needs, please notify the Health Office as soon as possible.
Medication Administration

Medication Administration Policy
Sometimes your child may need to take medication during the school day. The following procedures must be followed to ensure safe and proper medication administration at school:

A written order from your Health Care Provider is required for all medications, including both prescription and over-the-counter drugs (e.g., Tylenol, Advil).

Medications must be brought to school in their original, labeled containers. Loose or repackaged medications will not be administered.

Before sending any medication (such as nebulizer ampules, pills, EpiPens, inhalers, ointments, or eye drops), please check the expiration date to ensure it will remain valid throughout the school year. We cannot administer expired medications.

Self-administration of medication is not allowed, except for inhalers (for asthma) and EpiPens (for emergencies).
Consent Forms:

Both the Health Care Provider and parent must complete consent forms for medication administration.

These forms are kept on file in the Cornwall Central High School Health Office and are valid for one school year only.

Parents must provide updated doctor’s orders and permission forms each September dated after July 1 every year.

At the end of the school year, parents must pick up all remaining medication or make arrangements to do so. Any medication left after the last day will be discarded.

The School Health Services Program plays an important role in supporting your child’s academic success by promoting health and wellness within the school environment. As part of this commitment, we conduct state-mandated health screenings during the school year.
Screenings for Cornwall Central High School (Academic Year):

Scoliosis Screening – Required for Grade 9 males only

Vision Screening (Distance and Near) – Required for Grade 11 students

Hearing Screening – Required for Grade 11 students

Every two years, the New York State Department of Health selects school districts to participate in a Body Mass Index (BMI) survey. This is part of a statewide effort to monitor and support student health.
Here’s what families need to know:
What is BMI?
Body Mass Index (BMI) is a number calculated from a person's height and weight. It is used to screen for weight categories that may lead to health problems.Important Note:
Your child is not measured for BMI at school as part of this survey. BMI measurements are only done by a healthcare provider during your child’s annual physical exam.When is BMI Calculated?
BMI is calculated during your child’s annual physical exam with their pediatrician.
Note: School nurses do not calculate or collect BMI data.Where is BMI Measured?

New York State samples students in grades K, 1, 3, 5, 7, 9, and 11.
The data submitted comes from physicals done during the previous school year.How is BMI Data Used?
When your child hands in their physical exam documentation, the school nurse enters the information into the school health program. This program generates the BMI report used for the survey.No names or personal identifying information about your child are used for BMI reporting. Only height and weight measurements are included, and the report only displays BMI percentile information—no personal details are shared or given in this report.
The data is grouped by BMI percentile ranges:

33% of students: 0–5th percentile

18% of students: 5–49th percentile

11% of students: 50–84th percentile
-
21% of students: 85–94th percentile
-
17% of students: 95th percentile or higher
No individual student data is shared. All information is reported anonymously to the state.

Immunizations

Important Immunization Requirements for School Attendance
All students must submit proof of required immunizations before they can enter or attend school.
-
New students have up to 14 days from the first day of school to receive their vaccinations or begin the vaccination series. A schedule for completing any remaining doses must be in place.
-
Students transferring from out-of-state have up to 30 days to comply.
-
Students who have received at least the first dose of all required vaccines and are following a provider-approved schedule for additional doses may attend school while considered "in process."
If immunizations are not up to date within the 14-day window (or 30 days for out-of-state transfers), the student will be excluded from school until proper documentation is provided.
Students who do not meet these requirements will be considered out of compliance and will not be permitted to attend school until the Cornwall Central High School Health Office receives updated records from a licensed health care provider.

Physical Exam Requirement for Students
In accordance with New York State Law, all students in grades 9–12 who try out for a school sport, new entrants to the district, and students in 9th and 11th grade are required to have a current physical exam on file with the Cornwall Central High School Health Office.
Physical exams are valid for one year from the date of the exam.
If a physical exam is not provided by a private physician, the district’s Nurse Practitioner will conduct a school physical.

Working Papers for Students
Students who plan to work during the school year or over the summer are required to have working papers, in accordance with New York State Labor Law.
Who Needs Working Papers?

All students aged 14–17 who wish to be employed.
How to Apply: Step-by-Step

Start in the Main Office
Pick up a Working Papers Application (Form AT-17) from the Cornwall Central High School Main Office. -
Complete Part 1 of the Application
Fill out your personal information and a parent will sign where indicated. -
Go to the Health Office
Bring your application to the Health Office, so the nurse can verify that you have a current physical exam on file (within the past 12 months).-
If your physical is not up-to-date, you will need to submit an updated physical to the Health Office.

Once confirmed, the school nurse will sign the application.

Return to the Main Office
Bring your signed application back to the Main Office, where your official working papers will be processed and issued.

Sports Registration Information
To sign up for school sports, please use the link below to access our Athletics page and complete the online registration through Arbiter Sports.

We require all student-athletes (Varsity, JV, Freshman, and Modified) to register through this online system.
All required information must be completed in Arbiter in order for an athlete to be cleared for participation.Physical Examination Requirement
In order to participate in any tryouts or in-season practices, a current physical exam must be on file and reviewed by the school nurse.

Physicals are valid for 12 months.
